Maybe i can share some of the cost/itinerary on my previous trip to Ahfatt kelong so that those interested can have a rough idea.

Cost (as of april 2009)
Van (Round trip to/from SG) - RM700 (best if you can get about 10-12 people to share)
2D1N -> RM155

Itinerary
Day 1
7am - depart SG (stop by along the way for breakfast)
10.30-11am - Reached the malaysia jetty.
12pm - Reached Kelong. Had lunch.
4pm-6pm - Ferry to bring you to sibu island.
6pm - Dinner
10pm - Supper

Day 2
7am - Breakfast
11am - Lunch
12pm - Ferry to bring you back to jetty.
1pm - Van to pick you, reach SG at about 4+pm.
